Certainly true of the Space Shuttle Challenger disaster, when the engineers at Thiokol warned that the booster O-rings were not designed for cold-weather launches just to be overruled by management who approved the launch.
Am I going crazy, or did the exact same train of thought doom the Titanic itself? Weak rivets, sub-par iron hull plates, leave off half the lifeboats, etc?????
